Application Process and Required Forms

Applicants need to complete all of the following requirements in order to become a volunteer at UNC Hospital.  Please note that the steps which need to be taken to become a volunteer are listed in the order that they must be completed.

 

ADULT VOLUNTEERS

  1. Complete the Application
  2. Give the Adult Volunteer Reference Form to an employer (past or present), mentor, colleague, etc. (have them complete this form and send it to Volunteer Services.)
  3. Authority for Release of Information Form
  4. Bring the above forms to the Volunteer Services office or mail them to:

  5. Have a personal interview
  6. Attend the orientation
  7. Schedule and receive results for a TB skin test (it takes 2 days to process results)

 

COLLEGE VOLUNTEERS

  1. Make sure you meet all of the requirements listed on the main 'College Volunteers' page
  2. Schedule and receive results for TB skin test (it takes 2 days to process results)
  3. Complete the application
  4. Complete the Immunization and Tuberculosis Review Form
  5. Come to walk-in registration on the date listed on the main 'College Volunteers' page   -  *Applicant must have a TB test less than one year old prior to registering and must also bring proof of negative TB to registration!
  6. Attend the orientation
  7. Complete the online tests
  8. Come to the Volunteer Services office, purchase a polo shirt and get your ID
  9. Volunteer!
