Application Process and Required Forms

Applicants need to complete all of the following requirements in order to become a volunteer at UNC Hospital.  Please note that the steps which need to be taken to become a volunteer are listed in the order that they must be completed.

1. Make sure you meet all of the requirements listed on the Main 'Adult Volunteers' Page
2. Complete the Application
3. Give the Adult Volunteer Reference Form to TWO people (either an employer (past or present), mentor, colleague, etc.) - have them complete this form and send it to Volunteer Services
4. Authority for Release of Information Form
5. Bring the above forms to the Volunteer Services office or mail them to Suzanne Davis at:

6. Have a personal interview
7. Attend the orientation
8. Schedule and receive results for a TB skin test (it takes 2 days to process results)
